When to use it
Use this when your home has an ongoing issue — such as mold, no heat, unsafe wiring, or a pest infestation — that affects its livability and hasn't been resolved through a standard repair request.
What to include
- Your name, the rental address, and the date
- A clear description of each condition and how it affects your health or safety
- A timeline of when you first reported the issue and any responses you received
- A request for repair within a reasonable time, per your state and lease
- A note that you may contact local code enforcement or a housing authority if it remains unresolved
- A request for written confirmation of next steps

- Dated photos or videos of each condition
- Copies of any earlier repair requests and the responses you received
- Any inspection reports, code violation notices, or medical notes related to the issue
- A log of dates and impacts, such as missed work or health symptoms
